Allegation Detail: The Health Facilities Division received a complaint on 2/5/09.
The complainant alleged the facility failed to protect the patient from abuse. The complainant stated that in --- ---- s/he was abused by a staff member. After the patient's treatment, s/he went into the kitchen to get a pastry. A staff member then came in and grabbed the patient's arm so hard that s/he dropped the pastry.
Findings Detail: An unannounced onsite complaint investigation was conducted on 2/18/09 by one state surveyor. Interviews were conducted, observations were made, policies, procedures, staffing schedules, personnel records and the medical record of the patient of the complaint, --- ----, were reviewed.

Allegation Detail: The complainant alleged the clinic failed to inform the patient the dose of his/her medication. The complainant stated s/he was receiving Epogen and was vomiting, having diarrhea, and nausea as a result. The complainant stated the clinic failed to inform the patient the dose of the Epogen.

Surveyor Findings:
Based on medical record review and staff interview, the facility failed to ensure patients received an initial nursing assessment upon admission for three (#2, #3 and #4) of 5 medical records reviewed. The failure to ensure pertinent medical history was obtained and a physical assessment performed by the registered nurse prior to initiation of dialysis treatment has the potential to result in inadequate clinical information in which to develop an adequate plan of care.
